I had the sense that he was enormously self-critical, and after hearing more about James’s history I wondered if some of his self-reproach was overdetermined by his mother’s preferences for his older sister when they were growing up. James felt that his mother was likely to see his sister’s accomplishments as superior to his, and that she seemed to enjoy spending more time with his sister than with him. He also seemed protective of his parents, particularly his mother. When James would talk about his lack of ability in comparison to his partners, I did not know how to evaluate this observation.

He would laugh awkwardly as I drew his attention to his contempt and helped him to see the ways in which he tried to avoid expressing this contempt. At first, he only reluctantly accepted it. He was giving up his cover story, and he was unfamiliar with this openness. We referred to his self-loathing in part as a Trojan horse that had allowed him to more palatably pass as a mother-lover while disguising the fury inside. I am always struck by the kinds of things that make my sympathies shift toward my patients.

However, it can also yield high rewards. I have had patients who burst into laughter when I bring up the possibility that I actually could be yawning for a reason other than their impact on me. It is also important when a patient’s self-criticism creates a feeling in the analyst that there is no way to help.
